Free Youth Physical Vouchers

ConvenientMD offers youth pre-participation physicals for k-12 students at all of our urgent care clinics. You can learn more about our youth physicals at this link. We can provide your school with vouchers that waive the fee, and these vouchers can be distributed to any families with a financial need. Please note that the voucher only covers a youth physical - it does not cover an illness/injury visit, nor any ancillary services (e.g. immunizations, labs, etc.). Also, our clinics offer pre-participation physicals (for school, sports, summer camp, etc.), which are not intended to be a replacement for an annual wellness visit with a PCP.

      Narcan Community Donation Program

ConvenientMD used to assist schools in obtaining free Narcan through the national Community Donation program. However, the program no longer requires a provider to sign off on the request, so schools can request free Narcan at this link.

      Free Care

When requested by a school nurse, ConvenientMD can waive fees for a child in need of care. This care is only for children from families that have a genuine need (with no access to care and self-pay is cost prohibitive). Requests for free care are evaluated on a case-by-case basis - we do our best to ensure every child is able to receive the care they need, but we may not be able to accommodate every request. Some services require use of a third party (e.g. certain labs), and we are not able to waive fees from third parties. If this request is urgent, please call your local clinic and mention the School Nurse Partnership Program to the Practice Manager.
Please note that the parent/guardian only needs to sign the form if the school nurse wants notes from the child's visit.

        Miscellaneous Requests

Please include the number and type of stock medication(s) being requested (e.g. two albuterol MDI inhalers). This section is only for requests for prescriptions and not the medications themselves. If you are looking for free EpiPens or Narcan, please refer to the sections above. Prescriptions will be made out in the name of the school. School nurses can attempt to have these filled at a pharmacy, but please be aware that many pharmacies have policies against filling prescriptions for entities.
Indicate here if you are requesting something that is not already included on this page, or if you have any questions.
